Agree on requirements and weights before reviewing proposals. Rate each criterion from 1 (weak) to 5 (strong), document evidence, and apply mandatory gates separately from the score.

Name the decision, sponsor, evaluator and decision date.
Weights must total 100%. Define what a high and low rating mean.
Record each vendor rating, evidence source and unresolved questions.
Compute sum(rating × weight) / 100. Recheck after plausible weighting changes.
Document pass/fail requirements, tradeoffs and approval.
